Create a simple PDF form
Build an interactive PDF form from a list of text fields and checkboxes, ready to distribute and fill in any PDF reader. 100% local processing in your browser — your data never leaves your device.
How it works
Each field you define is laid out vertically on the page with its label above, then a real interactive form field (text box or checkbox) is added to the document, recognized by all common PDF readers. The form automatically extends across several pages if the field list is long.
What is a field's "technical name"?
It's the internal identifier used by the PDF (and by other tools, such as autofill or data export tools) to refer to that field, separate from the label shown on screen. Preferably use short names without spaces or accents, for example full_name rather than "Full name".
Can I add dropdown lists or radio buttons?
Not in this version of the tool, which deliberately sticks to text fields and checkboxes to keep things simple. For more complex forms, dedicated PDF software is still the better choice.
100% local processing: the form is created entirely in your browser — no entered data is sent to a server.
